This resource is a Word version of the form that needs to be filled out when challenging a library book. Make sure you read the instructions in the form so it is filled out correctly. You will only be filling out page numbers 4-5.
Once completed, save as a .pdf and email it to the school principal and CC: cfontenot@ccdfusa.com so we can add it to our list of challenged books.
